Primary Care Trust
The Horsham and Chanctonbury Primary Care Trust no longer exists. It has become West Sussex Primary Care Trust (PCT). A transition site for the new PCT has been created http://www.westsussexpct.nhs.uk/ and is still being developed.
The most up to date local information on services such as GPs and dentists can still be found on this site, however please check the transition website first for policies and procedures and if you cannot find what you are looking for, please refer back to your local site.
