Looking after your physical and mental wellbeing is vital. There are lots of organisations in Horsham District that focus on providing health help and advice, including support for young people.

  • Age UK West Sussex Brighton & Hove

    Age UK West Sussex is an independent local charity working in the community to support older people, their families and carers.  We want everyone to be able to love later life.

  • Alcoholics Anonymous

    An organisation that offers help to people with drinking problems.

  • Alzheimer's Society

    We are Alzheimer's Society. We're a vital source of support and a powerful force for change for everyone affected by dementia.

  • Billingshurst United Reform Church

    The church runs a monthly dementia cafe on the second Saturday of every month.

  • Communities of Practice (North Horsham)

    A team who work with people with long term conditions and their carers to actively promote health and wellbeing in the community; and where possible prevent admission to unplanned care/hospital.

  • Community Neurological Rehabilitation Team North

    We provide specialist therapy and support for people at home, who live with a neurological condition. We provide a co-ordinated service with specialist staff who will support your recovery, optimise your abilities and improve your quality of life, including your families and/or your carers.

  • Community Reablement Service

  • Headway ABI

    Headway is the UK-wide charity that works to improve life after brain injury. It provides support, services and information to brain injury survivors, their families and carers, as well as to professionals in the health and legal fields.

  • Horsham District Council Wellbeing Hub

    We can help you to find local wellbeing information and services. You can also get support for things like getting your family fitter, doing some regular exercise, dealing with stress, kicking a habit, or simply improving your general wellbeing.

  • Independent Living Service

    Occupational therapists are focused on supporting children and young people to achieve their own personal goals, and in facilitating carers to assist children and young people with daily living tasks. Referrals through West Sussex County Council Sharepoint

  • Intermediate Care

    We provide short term rehabilitative support to help individuals recover from illness or injury. This is achieved by agreeing goals with people and by helping them to become as independent as possible in their own homes. Subject to assessment by the social services assistant care manager, the person may have a package of homecare provided. The service is initially for a 2 week period and may be extended for up to a further 4 weeks in order to achieve their goals.

  • Mary How Trust for Cancer Prevention

    The Mary How Trust provides health screening which aim to spot the early signs of life-threatening illnesses such as heart disease, diabetes, bowel cancer, prostate cancer, and abdominal aortic aneurysm, on a self referral basis to asymptomatic people. Instead of a fixed fee we ask for a donation.

    A service committed to supporting the armed forces community in West Sussex, including serving and former members, reservists and their families.

  • MIND in Brighton & Hove

    Peer Mentoring in Crawley, Horsham and Mid Sussex. We provide Volunteer Peer mentors who will meet with you weekly for 12 weeks. Our mentors have lived experience of mental health issues and can support you to work on goals such as improving confidence, developing new skills or social activities.

    Child abuse is preventable - not inevitable. And it’s up to each of us to do everything possible to keep childhood safe from abuse, so children can grow up healthy and thrive.

  • One Team North

    One Team is an integrated multi-disciplinary team that provides rapid assessment and clinical support to prevent admission to hospital for up to 72 hours. Patients accepted by One Team North will be experiencing an acute alteration in their physical wellbeing which, without the input of this service, would result in acute hospital admission.

  • Pathfinder West Sussex

    Pathfinder West Sussex is an alliance of organisations working together to enable people with mental health support needs, and their carers, to improve their mental health and wellbeing. Can provide support around housing, debt and benefits only if the service user is receiving support from Pathfinder with their mental health. Referral or Self-Referral accepted.

  • Pathfinder West Sussex: Staying well

  • Phoenix Stroke Club

    The Phoenix Stroke Club is a club in Horsham, West Sussex, for people who have suffered a stroke or other neurological disorder.

  • Prevention Assessment Team (PAT)

    Joint teams including staff from West Sussex County Council, Sussex Community NHS Trust, and Guild Care or Age UK West Sussex. We aim to help people living locally to stay healthy and remain independent. We normally work with people who do not receive services from specialist health care teams or social care services.

  • Pulborough Community Care Association

    Lunch clubs, community transport, Tai Chi, Wednesday club, toenail cutting, sight problems group.

  • Relate West Sussex

    Counselling for individuals, couples, families and children and young people, aged 5+. Also provide psychosexual therapy for those facing sexual problems in their relationships. Offer subsidised counselling.

  • Roffey Place

    Horsham District Council is working with local homeless charity Turning Tides to develop and open a new innovative service to support people who are experiencing homelessness. The new service is at Roffey Place in north Horsham and will provide high-support accommodation for up to 50 people alongside a range of on-site facilities.

  • Salvation Army: Hope In

    Hope In community programme and drop-in is for individuals or families who need practical help and support and a listening ear, especially in the area of food poverty. A hot meal is available.

    Crawley and Horsham Stroke Recovery Service provides high quality information, practical advice and emotional support following a stroke. Whether you are a stroke survivor, carer or family member, we will begin working with you after a stroke.

    The Sussex Mental Healthline offers 24/7 listening support, advice, information and signposting to anyone experiencing difficulties, or who may be in crisis and in urgent need of help, with their mental health.

  • Tapestry Day Club

    Dementia day care provided in small groups (max six guests) within the family homes of hosts, who provide refreshments, including a two-course, home-cooked lunch, as well as optional, memory-boosting activities. Transport is also generally available if needed. Tuesdays-Thursdays. 01403 610345

  • The Hub Beeding

    A church that offers all-age community support in the communities of Upper Beeding, Bramber and Steyning.

  • Time to Talk

    Talking therapies, self referrals accepted, offering support with carious issues including management of long term conditions.

  • Youth Emotional Support (YES)

    Youth Emotional Support (YES) is a free service for young people aged 11-18 looking for support with their wellbeing. You must be referred through the single-point-of-access.
